As a team of healthcare professionals, we see the need and susceptibility of vulnerable individuals within our healthcare system and an overworked workforce. YOUR-Healthcare aims to bridge the gap between the general community and healthcare sector by limiting hospital re-admissions through early interventions or as they arise.
